CHURCH & OTHER EVENTS OF INTEREST

National Healthcare Decisions Day
Five Wishes Workshop

The United Church of Clinton

109 Haskell Avenue
Clinton, MA 01510
Tuesday, April 16th at 1:00PM
Workshop Led by Rev. Lee Atherton

National Healthcare Decisions Day exists to inspire, educate and empower people about the importance of advance care planning. Among other things, NHDD helps people understand that advance healthcare decision-making includes much more than living wills; it is a process that focuses first on conversation and choosing an agent.
Come for a Five Wishes workshop; the advance care planning program that is credible, simple and legal. It's more than just a document. Five Wishes is a complete approach to discussing and documenting your care and comfort choices. It's about connecting families, communicating with healthcare providers, and showing your community what it means to care for one another.
Five Wishes is unique among all other living will and health agent forms because it speaks to all of a person's needs: medical, personal, emotional and spiritual. Five Wishes also helps to guide and structure discussions with your family and physician, making conversations easier.
Five Wishes is the beginning of a conversation. Join us in conversation on Tuesday, April 16th at 1:00pm at United Church of Clinton. Please RSVP to Lee Atherton – 508-308-7116.  Suggested donation $10. FLYER
